The Request an Appointment form below is for appointments in all UT Dentistry clinics at our medical center location. Our clinics provide a range of care from experienced dentists and specialists, advanced graduate residents or dental students. A care coordinator will help you make an appointment with the clinic and provider that best fits your needs.

For patients interested in receiving care in the Student Clinic, please select ‘Student Clinic Screening’ in the form. The Student Clinic requires a mandatory screening appointment to determine if a patient’s treatment requirements are appropriate for student care. Due to scheduling limitations in this clinic, screening appointments require a $50 payment at the time of the appointment booking. This payment covers the cost of the screening and associated radiography. Dental insurance is not accepted in the Student Clinic.

Appointments are scheduled during normal business hours, Monday through Friday, 7:30 a.m. to 5 p.m. Seek immediate treatment at the nearest emergency room or call 9-1-1 if your symptoms are life-threatening, such as:

Difficulty breathing,
Uncontrollable bleeding,
Severe swelling or
An inability to swallow.
